Emergency Contact Card

Emergency Phone numbers and cell phone numbers are mandatory.  Please note any change of address or telephone number immediately to the School Office so that we may update our records for your child’s safety.  If you change your employment please notify the School Office with the name of business, address and telephone number.  This is necessary in case of an emergency.
